The vehicle is registered in [XXX] and was being driven to [XXX] when it suddenly overheated on XXX near [XXX]. The warning lights appeared while the vehicle was traveling at highway speed. The driver immediately took the first available exit within about one minute.  The failed system appears to be the engine cooling system, specifically the water pump. The vehicle is currently at Kendall Lexus of Eugene and should be available for inspection.  This failure created a safety risk because it happened suddenly on a major highway. The driver had to exit quickly and the vehicle became disabled far from home. A sudden coolant-loss or overheating event at highway speed can leave drivers stranded in unsafe locations with very little warning.  Kendall Lexus of Eugene inspected the vehicle. Their Service Manager stated in writing that the technician found a leaking water pump, misfires in every cylinder, and a 95% leak in cylinder #6 through the exhaust valves. She stated that the water pump failure caused the vehicle to lose coolant, overheat, and suffer internal engine damage requiring engine replacement.  Lexus Customer Relations has been notified under Case #XXX. Whether Lexus corporate has physically inspected the vehicle is unknown.  Before the incident, there were no known warning signs. The warning lights appeared five hours into the road trip when the overheating occurred. After exiting, the driver let the vehicle cool for 30 minutes, checked for visible leaks, and tried to continue only briefly around the block. The warning returned almost immediately, so the vehicle was towed.  This vehicle has extensive Lexus dealership maintenance history. Lexus quoted about $20,000 for engine replacement. I found online that a lot of people have the same problem for years 16-22 on the RX. I found out where the dealers are telling people excessive heat melts the glue holding these adapters in place, and it eventually give way and break. Most people including me park outside, I don't have a garage, and it's summer, this is defective, and it should be covered my Lexus. Since it just happened yesterday, the vehicle wasn't inspected by nobody, there were no warning signs that this was happening either. The safety issue is when it hit me on the back of the neck while I was driving. I am getting it fixed this weekend by another service shop (because Lexus charges too much).  This was extremely dangerous because if the hood is not secure, it could open at any time while driving and cause an accident, especially at high speeds. There is the potential that if the hood does not lock when closed after it opens, that it could possibly fly open while the car is in motion. One year later, on [XXX] 2022, valve cover gasket oil leak was found at 57608 miles. Two months later, on [XXX], 2022, Timing Cover oil leak was discovered again at 58821 miles. Then two months later, on [XXX] 2022, oil leak was discovered again at 59864 miles. [All above was repaired by Thompson Lexus in Doylestown, PA under Lexus Powertrain Warranty.]  Now, two years and 15000 miles later, the car is currently developing the same oil leak again at 76500 miles. The car is out of warranty, the repair will cost $7000.  Toyota/Lexus should provide a longterm sustainable solution for this problem instead of advising repair shop to merely reseal the cover, which will eventually leak again. I even learned that the next design flaw in RX350 is the water pump. I contacted the Lexus company and reported on the issue. They offered me $2000 discount. This means they are aware of the issue.  I believe Lexus shall fix this issue since it can turn into a safety hazard. If customers don't notice the leak, or leave engine oil leaks unchecked, the engine could seize up, or even catch on fire. Thompson Lexus dealership confirmed this oil leak. This leak not just happened, but it has happened long ago at least 6 months prior to the time that I discovered based on old oil dirty debris deposits and paints popped out and rusted caused by prolong oil on the frame. The reason that did not detect the oil leak right away was that I have almost not used the car at all for the past two years due to Covid situation. Second, I have parked my car on my lawn, so that could not see oil drops. Third, I put my trust on Lexus and Toyota products, so I have not inspected for oil leak periodically. This oil leak has soaked many parts of the engine, frame, electrical connections, and possibly other moving parts such pulleys and belts. This can potentially create smoke or fire hazard and belt and pulley slippery which can lead to accidents. At the time of the oil leak, my car was under 54,000 miles and under 6 years, so it was still under powertrain warranty with is 6 years and 70,000 miles. I have contacted Lexus several times regarding this matter. I told them about location of the oil leak and when oil started leaking, my car was still under powertrain warranty but Lexus refused to pay for the timing cover repair. After doing research on RX350 from timing chain cover, I have found out that a lot of Lexus RX350 has developed oil leak from timing chain cover since year 2009 or 2010 model to 2016 year model. Some RX350 developed timing chain cover oil leak from a few thousand miles of first use to 38,000 miles. I WAS STUCK IN THE MIDDLE OF THE ROAD AND WAS UNABLE START IT AFTER A COUPLE OF TRIES.  CALLED FOR EMERGENCY SERVICE, LET LEXUS DEALER KNOW MY CAR WAS GOING TO BE TOWED TO THEM, AND SAT FOR ABOUT 15 MINUTES WHEN A GENTLEMAN CAME ALONG BEFORE EMERGENCY SERVICE REP GOT THERE. WHEN DRIVING AT NIGHT, THIS GIVES A PARALLAX EFFECT AND IS DISTRACTING. DUE TO A CHIP IN THE WINDSHIELD, THE OEM WINDSHIELD WAS REPLACED WITH ANOTHER OEM WINDSHIELD THAT ALSO DEMONSTRATED THE SAME ISSUE. OTHER 2016 RX MODELS AT THE DEALER EXHIBITED THE SAME EFFECT. LEXUS HAD A TEAM INSPECT THE VEHICLE.  LEXUS SAID THAT THE LACK OF THE PASSING INDICATOR TO SEE ALL VEHICLES IN THE BLIND SPOT WAS NORMAL FOR THE 2016 LEXUS RX 350. I HAVE DRIVEN THE CAR FOR 8 MONTHS.  I FIRST NOTICED THE APPARENT MALFUNCTION DRIVING BACK TO PA FROM FL.  I BELIEVE THAT THE CAR PASSING INDICATOR SYSTEM USES RELATIVE DIFFERENCES OF SPEED BETWEEN MY CAR AND THE CAR BEING PASSED TO ACTIVATE THE FEATURE.  FOR EXAMPLE, THE INDICATOR LIGHT WILL NOT GO ON WHILE I PASS A CAR A  MUCH GREATER SPEED THAN THE CAR BEING PASSED.  I WOULD GUESS THE DIFFERENCE MAY BE 20 MPH OR SO.  HOWEVER, IF I PASS A CAR SLOWLY OR AT A LOW DIFFERENCE OF SPEED, THE CAR PASSING INDICATOR LIGHT FUNCTIONS AS ONE WOULD EXPECT.  OF INTEREST IS THAT MY WIFE HAS A 2013 LEXUS RS350 WITH A CAR PASSING INDICATOR SYSTEM AND HER CAR WILL ALWAYS INDICATE A CAR IN THE BLIND SPOT REGARDLESS OF HOW FAST THE RS350 PASSES ANOTHER VEHICLE.  THE LACK OF INDICATING A VEHICLE IN THE RIGHT BLIND SPOT AT ALL TIMES IN MY VEHICLE IS DANGEROUS.  THIS SAFETY FEATURE  HAS POOR DESIGN.  THE 2016 WAS JUST INTRODUCED IN LATE 2015.  LEXUS KNOWS ABOUT THIS PROBLEM AND CALLS IT A NORMAL FUNCTION. I HAVE NOT YET RECEIVED AN ADEQUATE EXPLANATION FROM LEXUS WHY THE CAR PASSING INDICATOR SYSTEM IN THE 2016 RX350 WAS DESIGNED THIS WAY PURPOSELY.  I BELIEVE THAT THIS ISSUE IS A SERIOUS FLAW.  I HAVE ENCLOSED TWO DOCUMENTS I HAVE RECEIVED FROM LEXUS.
