FSD disengaged and I was forced to manually drive in pre-dawn darkness with a failed turn signal until the next charging stop. The fault then behaved intermittently: it cleared on its own, returned, cleared again, and I resumed FSD for much of the trip. Then, with roughly 100 miles remaining, the left turn signal malfunction warning returned while the vehicle was actively driving itself, again forcing manual takeover and leaving me without a functioning left turn signal in traffic. This is a serious safety defect on multiple levels: (1) A failed turn signal prevents me from lawfully and safely signaling lane changes and turns to other drivers. (2) The failure occurred WHILE the automated driving system was operating and depends on functional signaling to execute its own lane changes \u2014 yet the system had been driving with a component that failed audibly and without warning. (3) The intermittent nature means the vehicle can re-enter a hazardous state at any time, without warning, at highway speed. (4) I cannot get a service appointment immediately, so I must continue operating a vehicle with an unrepairable intermittent signal failure, exposing me to citations and collision risk. This vehicle has an extensive repair history including body panel adhesion defects related to Tesla\u2019s 2025 cant rail recall, and this electrical failure continues a pattern of defects across multiple systems. No warning preceded the pop. The failure is reproducible in the sense that it has recurred three times in one day. Service records and buyback correspondence with Tesla are available on request. According to Tesla's own accident data, I did not disengage FSD. There was no audible warning, no effective visual warning, no takeover request, no controlled deceleration, and no safe handoff to the driver before the vehicle stopped controlling itself.  Within about one second of FSD becoming unavailable, the vehicle left its intended path and struck a concrete highway divider. If the divider had not been present, the vehicle likely would have crossed into oncoming highway traffic, creating the risk of a catastrophic head-on collision with other vehicles, including a semi-truck.  Tesla's accident report confirms I did not disengage FSD and shows no warning or takeover request before impact. I possess documentation supporting that FSD became unavailable without a safe transition to the driver.  This failure occurred immediately after Tesla serviced the vehicle and represented that the FSD system was functioning properly. Despite the accident data, Tesla Customer Support and the Service Center have refused to acknowledge or investigate what appears to be a failure of the FSD system's fail-safe behavior.  The failed component appears to be the FSD/Autopilot system and its driver handoff logic. The vehicle and electronic records are available for inspection. An automated driving system should not terminate operation at highway speed without providing adequate warning, a safe transfer of control, or controlled deceleration. I believe this presents a serious safety defect that could result in fatal crashes. FSD has regularly failed to respect speed limit changes, even though it seems to recognize / observe the speed limit signs. I am certain it\u2019s observing the signs because it renders them on the in-car display, but it doesn\u2019t actually adjust its speed in response to those signs. The most egregious of cases have been when driving through Texas, New Mexico, and Arizona where the speed limit is often 75 mph on Interstate highways (e.g., [XXX]). On multiple occasions the vehicle has ignored construction speed limit changes to 55 mph, and instead has tried to maintain 75-80 mph through the construction zone. Not only could that get me a nasty speeding ticket, but it could also put construction workers lives at risk. It has also happened in non-construction zones, like when on a non-interstate highway that passes through towns and reduces speed when doing so. I am requesting that NHTSA review this as a potential unintended movement / unintended acceleration / park-to-drive activation / rollaway safety defect. Tesla should be required to investigate the vehicle logs and determine whether the movement was caused by a software issue, gear selection issue, Autopark/FSD/Summon/mobile app activation, brake hold/parking brake failure, sensor/occupancy issue, accelerator pedal issue, or any other defect. This removes all rear visibility at the exact moment when the driver needs full awareness for lane changes, merging, and turning.  This behavior cannot be disabled or overridden. It is part of the default software design. As a result, the driver is left without any functional rear visibility method during critical maneuvers. This creates a clear safety risk and raises concerns regarding compliance with FMVSS 111, which requires reliable rear visibility.  The loss of rear visibility during lane changes and turns increases the risk of collisions with vehicles approaching from behind. Since the physical mirror is unusable and the digital feed is removed by the software, the driver has no safe or compliant way to monitor rear traffic during these maneuvers. The cross sectional area of each spoke is minimal too for aesthetic purposes increasing the stress field in the spoke. This is a cast aluminum wheel taking the shape of a forged or wrought product. Furthermore the wheel covers make it difficult to identify cracks in the wheel since there is no visibility allowing a driver to unknowingly drive on a wheel that has had one or more spokes fail. Because the seat belt does not function as designed, the safety of a child occupant is compromised, particularly in the event of sudden braking or a collision. This involves a primary safety restraint system.  The issue was discovered during normal driving conditions and is repeatable and consistent. The seat belt assembly is available for inspection upon request.  No warning lights, alerts, or messages appeared prior to or during the malfunction. The issue was not caused by an accident, impact, or misuse. The vehicle is otherwise operable.  The manufacturer was contacted for service. Initially, a service charge of approximately $55 was proposed to inspect or repair the seat belt assembly. After questioning whether a safety restraint system should be subject to a service charge under warranty, the charge was removed. The design of the electric doors required passengers who are not familiar with the vehicle to find the location of a manual release hidden from view beneath a panel to remove that panel and pull the manual release mechanism. There are no instructions present to explain to a passenger how to access the manual release.     The truck has no exterior door handles. The only means to open the door from the outside was a button located at the base of the B&C pillars. When the electric system failed, that door release button became useless. A rescuer arrived but had no way to open the passenger side doors from the outside. The truck\u2019s \u201carmor glass\u201d windows made forcing entry extraordinarily difficult. The rescuer was able to break the passenger side window with repeated strikes and attempted to rescue [XXX]  by extracting her through the front passenger window but was unsuccessful due to the heat from the fire. She was forced to retreat back to the rear seat. She survived the initial collision with no injuries and was fully conscious. She was unable to escape the Cybertruck and was killed due to smoke inhalation and thermal injuries.   The failure of the vehicle\u2019s design to provide emergency egress in the event of crash, power loss, and fire created a foreseeable and unreasonable risk of occupant entrapment and death in the event of a collision resulting in power loss. No loaner vehicle was offered, no workaround was provided, and I am still driving a vehicle Tesla knows is defective. In addition, the rear windows malfunction frequently. When I close the rear doors, the windows often jam or freeze. This concerns me because the Cybertruck is built entirely with electronic door systems\u2014there are no manual overrides. If power is lost during a crash, occupants may not be able to exit. This is even more alarming in light of recent reports of Cybertruck-related deaths. Multiple individuals have died in crashes where the battery was damaged or the vehicle caught fire. Power loss caused the doors and windows to become non-functional, and due to Tesla\u2019s armored glass, bystanders couldn\u2019t break the windows to rescue the victims. These people burned alive while trapped inside. This is not a theoretical issue\u2014it has happened. Despite these risks, Tesla has not issued a recall or urgent fix. I\u2019ve made multiple efforts to resolve these problems, but Tesla\u2019s delays, denials, and lack of proactive safety measures are deeply negligent. The vehicle was swerving all over the road and almost hit a curb and other parked vehicles at 25-30 MPH  Also, aside from this case in some areas on the highway the vehicle will cut over the lines and not stay in the lane while in FSD Autopilot.   I have NEVER had this issue where FSD Autopilot disengaged after a trip and returning. Once the settings are placed in FSD Autopilot they were never changed unless done manually.   There were no alerts/notification telling me FSD/Autopilot was disengaged. Conclusion: It has now been over seven weeks since the recall was issued, and no remedy has been offered. The drivers side did clean acceptably.  But not the passenger side!  I could see arcs on the windshield where the wiper attempted to drop fluid.  There were 8 or 9 such arcs left after the wiper withdrew.  I did see cleaning solvent near the outside of the windshield.  But I could not see out of the windshield.  Reported same to Teslas customer svc and a service visit was scheduled for Jan. 8, 2025. By this time I had manually cleaned the window and verified the solvent well was full.  Svc agent said, no one else complaining and system is operating 100% as designed.  Experienced same situation again on [XXX].  Customer svc again scheduled an on site svc call for Feb 7 2025. I then saw on facebook, over 50 other people complaining of exactly the same problem.  Most had tried svc and heard the same informational as I.  Some included photos of the problem, some had their wiper or pump replaced, other e told that Tesla was planning an OTA update to cure the problem. I did a walk-in to svc on Jan 27,2025, and was told there is no guidance from corporate on this matter and that as far as they knew, corporate was not aware of the problem.  I'll keep my appointment on Fri and hopefully I'll have some relief. I hate not seeing things coming from my right side! As this is a design defect and not a one-off part issue, I'd personally like to see this recalled. Tesla Service says it's due to the computer processor being located in that area with no way for the heat to dissipate (ie, a TV would have room behind it to dissipate the heat, but the Cybertruck has that area blocked).
