When the problem occurs, camera/vision functions and driving visualization become unavailable or degraded, and the vehicle has reported Self-Driving Features Limited, Cruise Control Unavailable, Adaptive Headlight Features Unavailable, and other driver-assistance limitations.  In Tesla Service Mode I have documented the primary Autopilot processor repeatedly changing between INIT, CRITICAL and COREBOOT while the secondary processor remains NOMINAL. A DAS reset can temporarily restore communication, but the failure returns.  Tesla has proposed replacing the entire HW3 car computer at a cost of approximately $3,120. I am concerned that this may represent a safety-related HW3 computer failure, particularly because I observed an ECU thermal alert and because the failure removes multiple camera-based driver-assistance and safety functions. This time, at 50,750 miles, your technician diagnosed the root cause and confirmed it is a system fault due to defective parts, and that it has been happening even when the car was under warranty\u2014not something I caused or could have prevented. He also told me directly that if left unaddressed, this will likely lead to compressor failure down the road.  Despite the issue being reported and documented while the car was under warranty, I've now been quoted $3,200 to fix it, entirely out of pocket, simply because the diagnosis happened to land a few hundred miles past the warranty cutoff. Had this surfaced further down the road as normal wear and tear, I'd have accepted it as a cost of ownership. But this is a known defect that was reported in-warranty and only went undiagnosed because of its intermittent nature \u2014 that shouldn't be the customer's burden to absorb.  On top of that, I found an uninstalled HEPA filter assembly sitting loose in my trunk after this service visit \u2014 not installed, not packaged, just left there. It had nothing to do with the nature of the visit. That's not the kind of quality control I'd expect, especially given the rest of this experience. Without warning and without any driver input, the vehicle autonomously activated an automated steering and driving assistance system, assumed full control of the vehicle, and did not return manual control to me despite my attempts to intervene and regain control of the vehicle.   The incident occurred during active rainfall and wet road conditions. The vehicle, while operating under its own autonomous system control without driver consent, drifted and struck the guardrail on the driver's left side. Following the initial impact, the vehicle continued moving and did not stop. The automated system remained active and did not disengage following the collision. The vehicle did not return control to me until a wheel separated from the vehicle.   This incident was not caused by driver error. The vehicle's automated system activated without driver input or consent, overrode manual driver control, prevented the driver from regaining control of the vehicle, caused a collision with the guardrail, and continued operating after impact until mechanical failure occurred in the form of wheel separation.   I am requesting a formal investigation into this uncommanded autonomous system activation as a safety defect. I had 2 accidents on the same day. I having been driving in the US from more than 5 years and never had a scratch on my vehicle nor even a parking ticket. i drove approx 600 miles in 2 weeks. So I was very comfortable in driving the tesla model y. Date of incidents - 07/07/2023. At Tinton Falls NJ. I was trying to take a K turn on a  flat road at around 9:30 a.m. I put the car in Reverse. When I pressed the accelerator pedal the car went forward and hit an Electric Pole. I then pressed the brake pedal . Made sure the car is on Reverse Gear . On leaving the break pedal the car was automatically moving forward towards the pole. This should never had happen. I put the car in Park mode and waited for 5-10 mins approx.  Then after some time when I put the reverse gear and left the brake pedal the car didnt move and on pressing acceleration it reversed. I parked my car at the site where I had my work. Police were called and they gave me a ticket.  I had a sprain in my neck. Later by 2 pm I drove with the same car Home at Edison NJ. (1 hr drive) I called Tesla Customer Service and they advised me to take the car to Springfield NJ dealership as it could be a software glitch. I decided to drive the tesla with my family in other car to the dealership incase they decide to keep the car. I was about to enter into Fadem Rd when lifted my feet off the acceleration pedal to slow down. The car all off sudden accelerated with no steering control. I noticed lots of car on the  street and a lady exiting out of near by Staples Drive Way. I grabbed the steering and rotated it all the way to the right to avoid any of them, hit a pole on the passenger side , pressing the brakes really hard and hitting a bush. The car stopped, no air bags deployed , no warning signs nothing. I was in a shock and pain .
