Autopilot and Full Self-Driving will not engage. The side repeater cameras and the rearview camera still display images normally on the touchscreen, so the failure is not a loss of video display. The loss of Automatic Emergency Braking and forward collision warning means the vehicle has no automated protection against a frontal collision, and the loss of adaptive headlights and auto high beam means the high beams no longer dim for oncoming traffic. I performed a touchscreen reboot and a full power-off reset, and the condition returned. The vehicle is equipped with HW4 hardware and falls within the model years covered by NHTSA campaign 25V002000 (Tesla SB-25-00-001) concerning a computer circuit board short, though my VIN was not included in that recall population. I am reporting this because a vehicle sold with these active safety systems is now operating without them, and I believe other owners of vehicles in this build range may be experiencing the same failure. The window was closing automatically (auto-lock), with no one touching the switch, when it pinched and injured my child's finger. FMVSS 118 requires power windows to detect an obstruction and reverse during hands-free closing. It did not reverse at all. This puts children and passengers at continued risk of finger and hand injury. Reproduced/confirmed by a dealer or service center? Yes I reproduced it myself. With a child-sized finger-width carrot placed in the window path during an automatic close, the window failed to reverse and crushed the carrot instead of retracting. I have video of this. Not yet confirmed by a service center; I am scheduling a Tesla Service inspection. Video can be viewed in the attachments that's in GIF format. Inspected by manufacturer, police, insurance or others? Not yet. I am reporting this to Tesla Service and filing this complaint at the same time. Warning lamps/messages/symptoms before failure, and when? None. No warning lamps, alerts, or messages appeared before or during the incident, and there were no prior symptoms that the automatic window reversal system was malfunctioning. The contact was very minor, and the resulting damage raised serious concerns about the safety and durability of the roof glass, particularly because I regularly transport my children in this vehicle.  On XXX, I brought the vehicle to a Tesla Service Center in Orland Park, Illinois. Tesla documented my report as a child/passenger safety concern. The technician verified an impact zone, documented that the top/exterior portion of the roof glass was cracked, that the interior section did not crack, and that a portion of the roof glass was pushed inward at the impact zone. Tesla recommended replacement of the roof glass.  I requested written confirmation that the vehicle was safe to continue driving with the damaged roof. After escalation, Tesla\u2019s Regional Manager responded that whether it was \u201csafe\u201d was a decision I would need to make at my own discretion and suggested replacement if I did not feel safe. He also stated that Tesla\u2019s glass is deemed safe for its intended use based on testing and government regulations.  My concern is not simply repair cost or coverage. I am concerned that very minor interior contact resulted in significant damage to a large glass panel directly above occupants, including children. The vehicle and its electronic data were preserved. Currently, non-inspection was done.   The unexpected maneuver placed the Tesla and surrounding motorists at risk because it occurred without warning at highway speed in active traffic and resulted in a collision. The driver had to intervene manually after the impact to control and stop the vehicle safely.  At the time of this report, the suspected malfunction has not been reproduced or independently confirmed by a Tesla service center or independent repair facility. No technical determination of the cause has been received. A police officer arrived immediatly after the accident.  An inspection was performed by Insurance company over a video.   Before the unexpected lane change, there were no warning lamps, messages, audible alerts, or other symptoms indicating that he vehicle was about to initiate the maneuver or malfunction. The lane change occurred suddenly while FSD was engaged. The impact damaged the front right body panel, front right bumper area, and the right headlight. My safety and the safety of vehicles in adjacent lanes was put at risk, as the collision occurred at roadway speed during an automated maneuver I did not initiate. Had the divider been a vehicle or a pedestrian work zone, the outcome could have been far more severe.  All FSD telemetry, camera footage, and vehicle data from the incident should be available from the manufacturer, and I have requested the vehicle data report from Tesla. The vehicle is available for inspection upon request and is being taken to a Tesla-certified collision center for repair. An insurance claim is being filed with my carrier. AAA arrived at 18:00 but could NOT tow because total power loss completely locked all 4 wheels/brakes (no Tow Mode). AAA dispatched a 2nd specialized flatbed with dollies (arrived 21:00). Driver was stranded on-site for over 5 hours. Car reached Tesla Dedham Service at 22:00. On XXX at 10:00 AM, driver reported the driving power loss in person at Dedham, handed over key, and got a loaner.  2. WRITTEN NOTICE (XXX): On XXX, explicit written notice was sent to Tesla Service Manager Taso, re-confirming the exact mid-driving power loss hazard.  3. TESLA DIAGNOSIS & REPAIRS: Invoice #XXX confirms a severe high-voltage failure: \"A hardware overcurrent event caused small high voltage battery brick imbalance and rear inverter failure.\" Tesla replaced Rear Drive Unit Inverter (3DU) and Pyrotechnic Battery Disconnect (Pyrofuse).  4. Symptoms & Alert Codes: There were zero warning symptoms prior to the incident. Immediately upon the power collapse, the touchscreen displayed critical faults. Active diagnostic telemetry confirmed alert codes:  DI_a137 (No Capable Drive Units)  DI_a063 (System Graceful Power Off)  3. Manufacturer Confirmation & Inspection Status: The failure was confirmed in writing by the manufacturer\u2019s service center. Sudden power reduction and shutdown while driving.\" The vehicle is currently held at the manufacturer's facility and is fully available for inspection.  4. With the vehicle powered off, the key card, Tesla app, and interior door releases did not function, leaving no way to open the doors from outside or inside. This represents a serious safety defect: a loss of vehicle power should not eliminate all means of entry/egress, particularly when children or pets could be trapped in extreme heat.
