Last year they reprogrammed the brake actuator assembly control module, and it worked for a while, but then once again after several months, it started again and the second time I took it in they drove it around to try to get it to mimic the problem, but was not able to so Couldn\u2019t do anything about it and just said to watch it and see if it became more frequent. They also said that once you have the actuator reset the computer shows updated and can\u2019t redo it.The problem with this issue is that it is random and sometimes it\u2019s less and sometimes more and I can\u2019t give specific dates because of the randomness of the problem. I have never not had it to stop, but I am including my two invoices from Toyota of Richardson where I\u2019ve had it seen as well as the research that I have looked up in different forums where other people are describing the very exact same problem with their vehicles. There is an issue, but don\u2019t know how to permanently solve it or know for sure what it is. The forum does explain some different things because of the vehicle being hybrid, but who knows whether that\u2019s it. The car sometimes does not brake as it should, meaning that the nose of the vehicle or the majority of the vehicle will extend past a stop sign or stop light, or contact in a parking lot could occur. The dealer was unable to reproduce the problem. The dealer inspected the vehicle twice. They turned the rotors (unrelated to the issue) the first time I took it in. The second time I took it in, I made them apply the T-SB 0047-24 software update. The T-SB 0047-24 update says that some drivers might be \"sensitive to the momentary transition from regenerative braking to hydraulic braking under certain driving conditions.\" The update to the software was designed to \"improve the feeling\" for the condition. The brakes are not slowing the vehicle predictably. You do not have to look far online to find the reported issues. If it can happen to normal people like us doing normal things and many other people like us reporting it, it can and will happen to anyone. A Vehicle less than 3 years old should not be leaking. I have spoken online to other owners experiencing the same.   But now since I see Toyota will not assist, and made it worse for us by improperly storing the Van and taking too long to respond. Now we cannot use it and it is a health hazard.  Instead of helping me, they instruct me to file a insurance claim I would otherwise not have had to file, if I just air dried the Van. Higher Trim has the \"digital rearview mirror\" option that could have alleviate this issue, but they don't even give that option to select as optional package on XLE trim with XLE plus and REAR SEAT ENTERTAINMENT package. I paid ~$4,000 for XLE plus and REAR SEAT ENTERTAINMENT package and it does include the \"digital rearview mirror\". This is a huge safety issue in point view and requesting NHTSA to take an action on it. I just googled it and found others complain for this hazards concern. They are just saying someone will call from nearest toyota dealership or reachout directly. When no call came, went to dealer who says reachout to TOYOTA directly! Its going in circle. Also reached out to DEALER from where it is purchased, they are too not responding. This is to be taken seriously. I am going thourgh lots of harrassment by dealer/toyota after paying this much for the car and should be compensated accordingly. NHTSA should help me getting DIGITAL REAR VIEW CAMERA installed in this vehicle and have TOYOTA to install same in all other similar configured TOYOTA SIENA vehicle. They all agree it is a major safety issue and reported it to Toyota corporate and opened a case to get it fixed. The fix is to install a digital rear view mirror for those who have the entertainment system (which again comes from the factory)  Toyota brand engagement said that yes this is a safety issue but since there is no ope recall they won't fix it for me by giving me a digital rear view mirror. This is absolutely crazy that there is this big of a design/safety flaw over looked by Toyota. They also told me that there have been some that have been fixed however it is a case by case basis and basically depends on who you talk to.  I just paid 52,000 for this car and I can't use the features it comes with because of this huge safety issue.  If I had my back window taped up completely blacked out and couldn't see through it, I would get a ticket from the police for a safety hazard of not being able to see out of my back window. How is this different? This is unacceptable and should immediately be recalled for those vehicles that toyota put entertainment systems in. This issue could cause people to be rear ended because they cannot see anything out of their windows. I can send pictures that the dealership took to verify this safety issue.
