I believe that both crashing into the lowered gates or crashing head-on into oncoming vehicles would likely have resulted in multiple deaths, including my own. Instead there is only a extremely small horn sensor on one side of the wheel that cannot be reliably accessed in an emergency situation b/c of its tiny size and b/c its position depends on where the steering wheel has been turned. This is EXTREMELY DANGEROUS. On a recent drive a car pulled into our lane from a side street and I was unable to locate the horn in a timely manner. Fortunately the car braked and I was able to pull around but it was very close and could have resulted in a head on crash.   I contacted Tesla about this issue. The service rep acknowledged the problem and said it was an \u201cissue\u201d that had gotten numerous complaints but that all he could offer was a Tesla aftermarket replacement that would cost nearly $1,000 to install.   I presume NTSB has gotten many complaints about this incredibly dangerous and egregious latent safety defect in the car\u2019s design. Tesla (at least for the 2024 and on Model 3 - which I also own) has corrected this design defect such that the center area does operate as a horn (as I am sure 99% of cars do).   The NTSB should investigate and require Tesla to retrofit any of its cars which have this safety defect. I have no doubt that people will (or likely have been) seriously injured (or even killed) in car crashes that could have been avoided had Tesla retrofitted its vehicles built with this dangerous safety design defect.   Please, please open an investigation and require Tesla to correct this inherent safety design defect.   Tesla already has the fix (and have implicitly recognized the defect by correcting it on its current models).   NTSB\u2019s failure to require Tesla to act will result in many avoidable accidents/injuries. The car\u2019s display immediately showed \u201ctire pressure 0.\u201d When I stopped and exited the vehicle, I found that the tire had either blown apart or peeled off the rim and separated entirely from the car. Weather was clear and around 70\u00b0F, road conditions were normal, and traffic was light. My tire pressures had been checked earlier that day and were all around 42 PSI.  I was unable to retrieve the tire because I have limited mobility and use a scooter. My daughter arrived soon after but could not locate the missing tire either. The vehicle was towed by AAA to a tire shop. The technician stated that the tire appeared to have peeled off the wheel, but could not explain why.  The tire was less than one year old and had very low mileage. I drive short local trips daily (around 10\u201315 miles) and had only taken one 700-mile trip earlier this year. Goodyear was contacted, but they declined to investigate because the tire could not be recovered.  This incident could have caused a serious crash had it occurred minutes later on I-25, where I planned to drive at 75 mph. Traffic is 30 MPH with a single lanes divided by a double yellow center line with cat-eyes. This area frequently has deer and other wildlife. Accordingly, whenever possible, high beams should be activated. Conditions were dry, and clear, without oncoming traffic -- while driving at or near the posted limit. Pushing and persistently holding the high-beam light maintained high-beam only while pressing the high-beam marked part of the steering wheel.  Typical operation, in the past, operated the high beam if the button (capacitative sensitive surface on the steering wheel) was held in for about a full second and then released. This is no longer how it operated tonight.    Driver pushed the high-beam for a full second. Upon release the high beams reverted to normal low beams. Driver repeated this 2 more times. After doing this twice, the driver is already distracted from wildlife and starting to focus on the car -- looking for error messages of some kind.  After three times, its clear that it is Tesla software that is broken.  Although the car is in good repair, no messages were presented. Again, conditions were clear and dry.  Failure to operate visibility mechanisms is highly contrary to safe driving. Some features which should always work: 1. Wind shield wipers; 2. Defogger 3. Defroster 4. head lights 5. high beams Hopefully, Tesla has not abandoned functional high-beams like they have with Full Self Driving. These tires are engineered by Michelin specifically for a Tesla Model S 2022+ hence the T2 mark. These are a second generation as the previous ones were a T1. Everything is supposed to be factored in by Michelin to cater to the vehicles specifications and needs.  I bought a set of 4 of these mentioned tires brand new from Discount Tire. In less 10000 miles and 7 months the sidewalls have completely decoupled from the tread section/rest of the tire. The treadwear itself shows and even smooth normal wear pattern. Discount tire and Michelin are trying to claim this is an alignment issue citing too much camber. This is a false claim. The area of separation is not in the contact patch with the surface of the road. This is a defect in engineering and construction in the tire itself. This calls for a mass recall.  I put the trust of my pregnant wife into these tires and she complained to me about a wallowing car ride with a decrease in handling, especially over bumps and on the highway. I could not see an issue. A few days later she informed me of a tire pressure indicator on the dashboard. It indicated tow low rear tires. Upon inspecting the tires I found both sides with a decoupled sidewall. Driver side being far worse than passenger. I immediately drove to Discount tire and the tire pressure sank from 26psi to 8psi in less than 4 miles. This could have proved fatal for my wife and unborn child on the highway.  The tire company and the stores dont want to accept accountability. The stores just want to sell more tires. I am having them replaced at a cost of $610 per tire. Thats equaling to almost $3000 spent on tires in 7-8 months.  Please investigate this matter. This is a defect. Not wear. The vehicle previously had Falken tires that lasted 24000 miles with smooth and even treadwear. Tesla's solution to \"mash\" the right controls on the wheel will activate the horn is terrible.  I usually wind up activating the wipers or voice recognition when I follow that suggestion.  Rear passenger door emergency release: The location of the rear door emergency release is located behind a small cutout of the rug behind your legs.  In case of emergency, time is wasted having to explain to find the cut out, open the cut out, look for a wire and pull.    No trunk emergency release:  That's right... There is no interior trunk release.  If you get stuck in the trunk, you cannot release/open it from the inside.  You will have to somehow fold down the rear seats and crawl into the back seats to exit. Noted loose and broken lower dash trim panel under the steering Yoke.  Upon investigation, noted the panel was designed to be held in place by 4 circular magnets, two on the panel and two on the lower dash support.   However, in this case, a 10mm  high strength steel bolt with a torque stripe was inserted into the mating magnet on the lower dash structure.  The strength of the magnet was such that the bolt was apparently fully seated.  The problem was, where did this bolt belong?  It was found the adjacent lower panel was the knee airbag panel was designed to be secured by two 10mm bolts, and one was missing!  The errant bolt was placed into the proper hole and torqued per the SM.  The passenger side panels and bolts were found to be properly installed, and torque striped.    I believe the placement of the Yoke makes it difficult for assembly personnel to ensure the airbag bolt is placed into the proper hole, and the magnet keeps the bolt in position when it is incorrectly placed.  The problem is this leaves the lower knee airbag secured by a single bolt, and that is a potential safety concern. The current Version is 2022.44.30.10.  For this reason, I have not reported these incidents to the manufacturer or made an appointment with the Local Tesla Service Center.  The problems continue even with the current software update. Until recently, I was unaware of others having these same issues or that significant injury, death, damage, have occurred. This is the deletion of the left stalk on the steering wheel to indicate a planned change of direction and blink the appropriate turn signals. Possibly one may memorize their location but until they are memorized the driver will have to take his/her eyes off the road for the few seconds to it takes to find the button. Neither of these buttons feel different by touch compared to the nearby surface. They can only be selected by looking at them. Of course if the steering wheel is not in its default position such as when the driver is making a turn, he/she has to take the eyes off the road for a few seconds. At 60 mph the car travels about 440 feet in 5 seconds.  I looked up the Massachusetts Drivers Manual, on the web, about the use of the directional signals. I copied those sections below. Even on the day we purchased the car (Sept 30) and drove it home (a short drive on highway 95) we had a similar incident but it was so new we weren't sure if it was us or the car but now we know it is a fault of the car and creates a very dangerous driving situation which could result in an accident or death to a passenger in our car or in the car behind us when our car decides to slam on the brakes for no apparent reason.  There were no warning lamps, messages or symptoms of the problem prior to the failure. We turned in a service request to Tesla and their response was to drive the car and have it happen again and create a Bug report.  So basically Tesla is asking us to put our lives on the line to help them diagnose the problem because they don't want to take the time to review the diagnostic data that is readily available to them. The entire user interface operates in a random way at random times in random traffic. This is a recipe for distraction. Tesla needs to encourage eyes-up, on the road. Instead the multiple stages of failure, result in many unnecessary risks. The car was/is made available to Tesla's service center, and they are aware of the contents of this report. In the video, I tap the slider that controls fan speed. Some 7 of 12 touches to the screen simply do not produce a corresponding effect: a) changing the slider positions; nor b) changing the rate of the HVAC fan. This is a safety risk, since a second attempt to touch the screen is required, demanding additional driver focus on the display, and away from watching for traffic and correct driving. This Tesla failure is a pattern observed across several user interfaces, including the voice command system, which is highly unreliable, even in quiet settings with normal N. American speech patterns, for which the car was presumably designed. Faulty car responsiveness across multiple systems is not a coincidence. It is a sign of cultural failure at the highest levels of Tesla to make their cars safe. No warnings, in the displays, or in the Tesla manuals, suggest that this would be a problem under any condition.  No inspection has been made by Tesla of this car. Apple does not have this problem, even with their earliest phones. It is unknown what positive purpose there can be to have the display software reject taps, even brief ones. I've had several incidents where I was unable to turn on my blinkers because the wheel had been rotated and the arrows were not only way out of their normal location, but they were also confusingly reversed in direction.  Instead of focusing on looking for pedestrians, watching other traffic, and properly starting the turn and/or exiting the roundabout, I have to focus on finding the turn buttons and figure out which one I need to use (since they're both facing the opposite direction in that wheel orientation).  Other incidents include turning on the wrong indicator when the wheel is rotation half way around, and not being able to find the indicators quickly enough to activate them in emergency situations (like when an ambulance appears and I need to quickly turn them on to pull to the side of the road).  Finally, unlike physical stalks, they give no physical indication of when they turn off, so they are often left on when the computer isn't smart enough to turn them off automatically, or when I don't think they're turning off properly, I press the button again to toggle it off only to have it turn off an instant earlier, causing me to turn on the signal again and possibly confusing other drivers. As a result, I have to dismiss the pop-up UI & avoid triggering the controls within it, by making a sliding motion on the center display, with my fingers applied to the UI. This is a problem because: 1) the UI displays the option to trigger one of my 3 garage doors, each of which have an associated homelink rectangular button. This is useless when the garage is already open. 2) I'm in reverse, so I'm trying to back-in to the garage in one motion.  Adding an extra steps, by clearing this obscuring UI distracts. 3) With tight clearances added problems are: trash might lie outside the garage; items can be left in my garage stall; people could be nearby.  Distractions mess this up. Tesla likely fails: a) that the garage could already be activated to be open (or even opening) -- & therefore, tends to prompt the user multiple times, while parking;  b) that 3 homelink garages are at the home. They didn't budget screen real-estate to show the homelink buttons without obscuring the backup display-screens assigned to each of the 3 rear-ward facing cameras; c) that a driver prefers to back-in to the parking space, relying on views of the 3 cameras to avoid obstacles.
