The vehicle has experienced repeated delayed or absent acceleration from a complete stop, creating a serious risk when entering traffic. Before the most recent repair, I reported intermittent hesitation to the Toyota dealer. On Sunday, July 12, 2026, shortly after startup, the vehicle hesitated twice within the first half mile. At the second stop sign, I attempted a left turn into traffic and applied approximately 30% accelerator pedal. The engine was running, but the vehicle only crept forward at idle-level power for approximately three seconds. There was no noticeable increase in engine RPM and no warning lights. Normal power then returned. On July 15, 2026, the Toyota dealer performed TSB T-SB-0017-24 and updated the engine-control software for hesitation during acceleration. After that repair, the same loss-of-acceleration condition happened again while my wife was leaving a parking lot. She was stopped at a stop sign, making a right turn into traffic on a slight uphill grade, when the vehicle failed to provide normal acceleration. This was substantially similar to the earlier incident and occurred after the software update intended to correct the problem. The repeated failure to respond to accelerator input from a stop creates a collision risk because the vehicle can begin entering traffic without providing the expected power. I am requesting that NHTSA investigate whether this is a broader safety defect affecting the Grand Highlander Hybrid MAX and whether the existing software repair is effective.
I've had consistent issues with the vehicle accelerating and shifting. These include sluggish acceleration, loss of power and sputtering when we've come to a complete stop and starting to move again. We've almost been hit by other vehicles at least over 20 times because the vehicle has failed to accelerate when pulling into a roadway. The car hesitates during acceleration regardless of how much I push the gas. Once it starts to go, it feels as though the car can't shift from 1st to 2nd gear, The RPMs get extremely high while the car is going 25-40 mph and remain there for a few seconds up to ~20 seconds. I am fearful of getting t-boned when my car hesitates while turning at an intersection, going through an intersection or pulling out from a parking lot. I have been stuck in several intersections when traffic was coming toward me, but thankfully those cars had patience and avoided us. I've also found that this issue always happens when the hybrid battery shows that it only has a charge of 3 bars. It seems that the engine, transmission and hybrid battery don't know how to communicate when the battery charge gets that low. This has been happening since Dec 2023 when I bought the vehicle and the dealer has never been able to duplicate or find any error code, they are refusing to fix a known issue when many others are experiencing the same issue. There are Toyota Technical Service Bulletins (TSBs) out there regarding this problem but my dealer told me they won't do anything as it could label my vehicle a lemon. This is creating an unnecessary safety risk for me and my family.
The passenger-side second-row bench seat (60 percent section of the 60/40 split) has a defective seat track/latch assembly that fails in both directions: it seizes completely when it should slide, and it failed to stay latched while the vehicle was in motion. The seat saw almost no use for the first 34 months of ownership (third row kept folded). Within two weeks of its first regular use (XXX, approx. 45,700 miles), it seized completely and could not be moved in either direction. An authorized Toyota dealership in Placerville, CA disassembled the seat and documented in writing that the seat track was twisted and rubbing against the seat frame. The dealer straightened the track rather than replacing it and stated on the invoice that if the seat stopped moving again, both seat tracks should be replaced and the seat frame itself may need replacement or adjustment. After that repair, in approximately mid-June 2026, the bench slid FORWARD on its own as my wife braked for a stoplight, the track failed to remain latched with the vehicle in motion. Two rear-facing infant car seats are installed on this bench, and our toddlers reach the third row through this seat. On XXX (approx. 46,416 miles, 33 days and about 700 miles after the repair), the seat seized again. The vehicle is currently at a second authorized Toyota dealership in [XXX] for diagnosis and is available for inspection upon request. Safety risk: a second-row seat that does not remain latched cannot properly restrain occupants or anchored child restraints during hard braking or a crash, and a seized seat blocks third-row access and egress for children, including in an emergency. There were no warning lamps or prior symptoms before either failure. Note: NHTSA recall 26V128 covers 2021-2024 Highlander second-row seats that fail to lock; this Grand Highlander is not included in that campaign but exhibits a similar failure to remain latched. The vehicle instructed me to pull over immediately and significantly reduced power output, making it difficult to maintain highway speed and safely maneuver through traffic. I was able to reach the shoulder and had the vehicle towed to a Toyota dealership. The dealer diagnosed a punctured radiator caused by a rock or similar road debris that allegedly passed through the front grille and struck the radiator. The dealer confirmed that coolant loss from the puncture triggered the vehicle's protective reduced-power mode. This failure created a significant safety risk. A sudden loss of power while traveling at highway speed increases the risk of rear-end collisions and other accidents, particularly in heavy traffic where drivers may not expect a vehicle to rapidly decelerate. There were no warning lights, messages, leaks, drivability concerns, or other symptoms before the incident. All warnings appeared suddenly at the time of failure. Toyota has declined warranty coverage, classifying the damage as a road hazard. However, I believe this may indicate a design defect. I have identified numerous reports from other GH owners describing nearly identical failures involving rocks penetrating the radiator through the front grille, often at relatively low mileage. These reports suggest the radiator may be inadequately protected from ordinary roadway debris. My concern is not simply radiator damage, but that a small piece of road debris encountered during normal driving can disable a critical component and cause the vehicle to enter a severe reduced-power mode at highway speed. These alerts included "Hybrid system malfunction" "All Wheel Drive malfunction," "Check engine. See dealer," and finally something akin to "Find a safe place to pull over and turn off vehicle." Until these alerts appeared, back-to-back-to-back, there were no indications that there was anything wrong with the car. We were fortunate enough to be both close to home and close to a dealership when this happened and were able to bring the car in right away. If we were in a high traffic situation, or, conversely, in a very rural area (specifically in winter, not an unlikely happening for us), having the car's drive system suddenly fail out of nowhere could have been very dangerous. As I said, we were able to get the car to the dealer soon after. The dealership informed us that the car has a coolant system perforation, more than likely from a rock penetrating the grill. The coolant than leaked, causing the hybrid system/other failures the car noted. I have since done some research on this issue and have found, to my dismay and disappointment, that this isn't an uncommon issue with the Grand Highlander and the Highlander. There are many instances of people on social media saying a similar issue happened with their cars - one other person's dealership quoted them the exact same amount of the repair that our dealership did. In fact, this is such a common issue that there is apparently already an established workaround: other owners have ordered aluminum mesh from third party vendors and have zip-tied it to their grills in order to keep the rocks out. This is an absurd thing to have to do. Consumers should not, in the year 2026, have to drive down the road with concern that a loose pebble will put their car out of commission. Cars should be able to withstand this very common occurrence without taking substantial damage. The dashboard then flashed a warning about the hybrid system overheating. The only way to stop it was to turn off the engine, then turn it back on. The car then began driving normally. I have also had an issue with hesitation felt while accelerating 0-40 mph with study throttle. Upon researching both issues, they should be resolved with TSB calibrations to my car of T-SB-0028-25 and T-SB-0017-24. This is not an error that would come up when doing a diagnostic check on my car's computer per research I have done. I took my car to get the updates at Toyota of Redlands, CA, and they refused to do them unless they could recreate them themselves. I called Toyota to report this, and they said that each Toyota dealership/service is privately owned at their own discretion. I was basically told to return if I experience it again. I do not want to experience this again and they wouldn't take my word on it anyways. I feel like next time I could injure myself or someone else if this glitch happened again. I therefore would like to implore to make this a mandatory preventative TSB calibration for cars that have had this reported/models/year which mine has. This goes for any TSB calibrations should be done preventatively, but especially ones that can cause a potential accident like T-SB-0028-25 and T-SB-0017-24. Here is information on why this would not show up on a diagnostic test: Your Toyota didn't generate an error code because the shudder is a software calibration issue rather than a component failure. Rapid window closure created a sudden spike in cabin air pressure — a documented pressure differential mechanism — that forced the moonroof glass outward until it shattered. There was no external impact of any kind. The glass, which is not laminated safety glass, scattered throughout the interior with occupants present. Component is available for inspection. This failure is consistent with NHTSA Investigation PE13-035 (Kia Sorento panoramic sunroof spontaneous breakage), in which ODI found glass shattering while in motion to be a safety hazard warranting engineering analysis. The same pressure-based failure mechanism applies here. This is a documented pattern on this specific model. Lemberg Law [XXX] ) analysis of NHTSA complaint data confirms spontaneous sunroof shattering among the top defects on both 2024 and 2025 Grand Highlanders, with Toyota denying warranty claims by alleging "outside influence" — disputed by owners and attorneys alike. The vehicle has been in the possession of Koons Tysons Toyota since December 23, 2025 (80 days as of 3/11/26), for repair of Safety Recall #25TA13 and #25TB13 NHTSA 25V744) involving the Panoramic View Monitor System/Parking Assist ECU. The dealership has explicitly informed me that the vehicle is unsafe to operate and have therefore kept it. Toyota and its authorized dealer have made three (3) repair attempts to correct this safety defect. Two separate replacement components were ordered and installed without resolving the issue. A third required component is currently on backorder with no definitive ETA or repair completion date, as the date keeps getting pushed. We are not being updated and are not receiving information from the dealership and Corporate about the details about the problem and nature of the delay. Toyota has obligations under federal recall law administered by the National Highway Traffic Safety Administration (NHTSA) to remedy safety defects within a reasonable time and at no charge to the consumer. An indefinite inability to complete a safety recall repair due to parts unavailability raises serious concerns under those obligations. When pressing the pedal to accelerate, the vehicle does not respond at all for several seconds. Then, without warning, it suddenly surges forward with excessive acceleration once the power engages. This problem happens intermittently throughout a drive each day, often enough to be concerning. It occurs after being stopped at an intersection or in traffic. When the vehicle finally responds, it accelerates quickly, which could cause me to lose control or collide with another vehicle. Safety Concern: This malfunction creates a significant safety risk. It puts myself, passengers, and other drivers in danger when the vehicle fails to accelerate as expected in high-risk scenarios such as crossing intersections, merging into traffic, or making left turns. The hesitation can leave the vehicle exposed in traffic, and the sudden lurch forward can cause unintended movement or rear-end collisions. The stated fuel tank capacity from the owner’s manual is 17.8 gal, however the maximum amount of fuel that can be pumped into the tank using auto fill at the gas station is 12 gal before the pump “kicks off”. After this, the fuel gauge on the dash reads full, however fuel range shown is 330 mi (at 30 mpg average gas mileage) when it should be closer to 500 mi with specified fuel tank capacity of 17 gal and 30 mpg. Shortly after filling the fuel tank, the fuel gauge and range will quickly decrease while driving. From researching and reading dozens of online forums about this issue, I tested filling the tank manually at the pump, extremely slowly, not using the auto fill lever. Two times doing this test, I was able to pump 14.5 gal into the tank before the pump “kicked off”. After these fill ups, the fuel gauge showed full and did not quickly decrease when driving. Also, the range now showed 450+ mi remaining. Both times after what appears to be an actual full tank fill up, the vehicle recorded over 400 miles before the fuel low light turned on, while previous max miles was in 300 range. It seems as if the fuel tank design does not allow the fuel to enter and settle in the tank quickly enough when filling, causing the pump to “kick off” prematurely and not actually fill up the tank. This issue causes the vehicle to not reach the full mileage range with advertised MPG and tank capacity, causing more frequent and inconvenient trips to the gas station. We have taken the vehicle to a Toyota Service center and they documented the issue but were told by Toyota Corporate that they do not have a fix for it at this time. In real life situation of stop and go, and needing to accelerate after making a right turn out of a driveway or stop sign on to flowing traffic, often times it feels as if the gear is shifting too high or skipping causing the car to lose power/ rpm and the vehicle chugs, before skipping back to the correct gearing and accelarating. I'm not sure how to describe it but I feel it is a safety risk especially if there are cars coming behind you. I have taken this into Hamer Toyota 2 times, but they can't seem to recreate the issue. However, my wife goes through this almost every day during her commute to and from work. Imagine driving a manual transmission and accelerating on 1st or 2nd gear, and all of a sudden the gear shifts to 4th or 5th gear, causing loss of power or rpm and due to natural instinct of depressing the accelerator further for power and the gear now shifting back and clutching grabbing at 1st or 2nd gear with the accelerator depressed further down than the beginning. This causes a jerking motion of our body in the car, because the car loses power, the all of sudden jerks back into power. I have attached URLs to people complaining about this besides myself. I'm seeing that toyota released a TBS to update software, but it seems its not readily available every time I call, or they need to recreate this before anything is done. I think this is a big safety concern and eventually cause accidents by getting rear ended. I'm sure other drivers think we can't drive because of not being able to speed up then rocketing forward. help! On another instance, I was at a stop sign and when I had the right of way, I stepped on the gas pedal but the car would not accelerate until roughly 10mph, at which time the acceleration kicked in. After this happened a few times, and after reading about it in social media groups, I brought the issue to the attention of the dealership. At the time of service, they were able to replicate the issue and performed a software update (T-SB-0017-24). That seemed to help but only for a few days, then the issue happened again - twice in the same afternoon. I took the car back, only to be told the issue could now not be replicated and that there was no additional fix. It happened again this morning. On [XXX], 2024, the rear windshield/liftgate glass spontaneously shattered without any apparent cause. The vehicle was merely six months old at the time of the incident. This unexpected and potentially hazardous occurrence has prompted me to investigate further, and I have come across multiple reports online from other customers who have experienced similar issues with the same rear windshield/liftgate glass on this specific vehicle model. This pattern of incidents raises significant concerns about a potential manufacturer's defect that may compromise the safety and reliability of these vehicles. Given the frequency of such occurrences and the potential safety implications involved, I am urging the National Highway Traffic Safety Administration (NHTSA) to investigate this matter thoroughly. It is imperative to identify any underlying issues or defects that may be contributing to these incidents to ensure the safety of consumers and prevent any potential accidents or injuries in the future. Please consider this complaint as an urgent matter, and I implore the NHTSA to take appropriate action to address this issue promptly. Your attention to this matter is greatly appreciated, and I look forward to a swift and comprehensive investigation into this concerning issue. Unfortunately, I have been noticing some issues with the hood (vibrating) at any speed which raised a huge concern about the safety of my family as it seems like the hood is going to fly away from the vehicle (at high speed like 50 mph or more). The formal response received from 2 different dealers is: We just got off the phone with the Toyota technical specialists about the concern with the hood. They have one other case open for the same complaint but are telling us there is no safety concern at this time and to release the vehicle back to you. We did test 2 other Grand Highlanders and they are both doing the same thing. My personal opinion is that due to the lightweight nature of the hood it is susceptible to flex and is showing that movement more obviously because of the black paint’s reflection properties. In my experience, if more of these cases arise, the manufacturer will open an investigation into a possible resolution but at this time we have been instructed to take no action. Thank you very much is all I got from them
The vehicle main screen on the dash will not go past the “get started” screen without accepting Toyotas privacy agreement on their app. I cannot use the screen as it defaults to this screen each time the car starts and does not allow a menu to the feature I paid for. This is both a safety and privacy issue as I should be able to use the vehicle while not agreeing to the privacy policy and I should be able to access the screen without distraction from driving. I explained to the salesmen I did not agree with the privacy policy and have been able to navigate the vehicle without issue in my 2014 and 2021 Highlander. I contacted Toyota and there is not work around. The manufacturer will not provide a solution other than removing all my safety features, which did allow me to get past the Home Screen. I asked to return the vehicle because I cannot use it and was refused. I would like to be able to protect my privacy and be able to use the screen I paid for without offering access to cameras and other location and audio recording devises on the vehicle.
