After normal use, a child who had been properly secured in a child booster seat in the rear passenger-side seating position was unable to be released from the seat belt. The vehicle had not been involved in an accident, collision, or any emergency situation. The child had simply been riding in the vehicle under normal driving conditions.  When it was time for the child to exit the vehicle, the rear passenger-side seat belt had become tangled and tightened around the child. The seat belt would not unlock or release despite repeated attempts to unbuckle it and allow the belt to retract or loosen. The restraint effectively trapped the child in the booster seat.  Because the child could not be freed using the seat belt release mechanism, the only way to remove the child safely was to cut the seat belt. This occurred solely because the seat belt could not be released after ordinary use, not because of a crash or any other unusual event.  A seat belt is intended to protect occupants while also allowing them to exit the vehicle safely after normal operation. In this case, the rear passenger-side seat belt failed to function as intended and created a serious safety hazard by trapping a child and requiring the seat belt to be cut in order to free them.  I am requesting that this incident be investigated to determine whether the rear passenger-side seat belt or its locking mechanism has a manufacturing defect or design issue that could pose a risk to other occupants, particularly children using booster seats. I should have to spend that kind of money on tires that are not worn, just poorly made. Falken admitted that they have a lot of complaints about this tire. Dealerships have stated that the behavior is normal, despite the repeatable nature of the issue.  No warning lights, error messages, or diagnostic indicators have appeared at any time. No diagnostic trouble codes have been reported.  The vehicle is equipped with an 8-speed automatic transmission, and the behavior appears consistent with a possible transmission, torque converter, or drivetrain control malfunction affecting acceleration response. I turned the car completely off as not to kill battery and started to goggle the RAV4. I attempted to restart the car numerous times. Called for a tow, prayed that I didn\u2019t get killed once traffic started to move. Finally 1-1/2 hours later it started. Traffic began moving 15 minutes after I got it started. Took in for service, charged $199.61 only to be told there were no codes. Problem could not be duplicated and in the future to always disable the start/stop button while using the vehicle.  October30, 2025 raining, I stopped at Panera drive thru. Placed an order waiting in line and car shuts down, same error code,  couldn\u2019t restart car for 20 minutes. Took to Toyota  November 5th same response no codes and cannot replicate issue, car is ok. The problem is the braking ability is failing for a split second, when the car hits a bump. The size of the bump can be as small as the crack in the asphalt where two roads meet. The problem occurs when I am driving at slow speeds and riding the brake because I am in a situation like being in slow-moving bumper-to-bumper traffic, approaching an intersection requiring a stop, or coasting down hill. On numerous occasions, the brakes release, allowing the car to surge forward for a split second, before re-engaging. Toyota has told me it is the ABS working as intended. To be clear, the braking failure is happening at slow speeds and on dry pavement. This is not how ABS normally works. I have driven numerous makes and models of cars for 40 years and I have never experienced this type of situation triggering an ABS system. I report this to Toyota each time I bring my car in for regular maintenance. During the last visit, my regular service technician told me that they have now seen at least 5 or 6 other cars doing what I have described above. The safety issue was loss of speed and braking control of the vehicle.  Steering was normal.  Fortunately the parking lot had few cars or people in it. 3.  No investigation of the problem aside from assessment of damage that I know of. 4. Liberty Mutual assessed the damage.  The NY state trooper made a collision information form and gave us a copy.   5.  No warning lights or related symptoms were noted prior to the car traveling on its own. Florida law defines \"impairment of value\" not by market collapse, but by any reasonable reduction in consumer confidence. Consumer disclosure laws require honesty about defects. Florida Statute \u00a7\u202f681.102(15) defines a \u201cnonconformity\u201d as a defect that \u201csubstantially impairs the use, value, or safety of the motor vehicle.\u201d My vehicle has been subject to repair more than 4 times for the same defect within 24 months and thus qualifies for relief. The manufacturer\u2019s failure to permanently correct the squeaky brakes\u2014even after attempted repairs\u2014demonstrates a pattern of unresolved nonconformity.  The squeaky brake issue is not a cosmetic inconvenience\u2014it affects the vehicle's functionality, resale integrity, and safety assurance. Toyota acknowledges the problem exists (via TSB) but has failed to provide a lasting solution. Over the next several days, I heard increasing thumping and metallic noises from the front right wheel area, especially at highway speeds. On [XXX], while driving with children in the car, the vehicle made loud metal screeching noises and the steering became impaired\u2014I could not complete a basic turn. I limped the car home at 4 mph. Upon inspection, I found two lug nuts missing and one so loose I could unscrew it by hand. I had the car towed back to South Coast Toyota on April 18. The dealership admitted two lug nuts were missing but failed to apologize or fully document the issue. I was told via text that the only action needed was replacing the missing lug nuts. I received no formal inspection report or confirmation that the vehicle was safe. I paid out of pocket for a third-party inspection at a separate Toyota dealership, which confirmed that the previously recommended sway bar replacement was unnecessary and that my tires and components were in better condition than initially claimed. This was a clear service-related safety failure that could have led to wheel detachment and a serious accident. I drove over 200 miles on freeways with children in the vehicle. The dealership failed to follow basic safety protocols and has not taken responsibility. I am reporting this to NHTSA in the interest of safety and accountability. The origin equipment tires that came with new vehicle were Dunlop grandtrek pt20. The new tires I got are Cooper Endeavor plus.  Issue is definitely not the tires. Typically this issue is a lot more subtle but occasionally it is so bad that I have a momentary feeling of loosing control over the speed of the car and active school zone is the least place I'd like that to happen. Besides, these stutters are pretty bad disturbers when they occur drawing focus away from the road and onto the car itself, since it does feel like the car is about to stall.  I have visited the dealership twice regarding this concern, however since these stuttering have a random nature I was not able to replicate the issue during my visits to the dealership. I have also passed this concern to Toyota USA Corporate office and they in fact have acknowledged that they received numerous complaints on the same matter and suspect the issue is in the newer 8 speed transmission system they are now installing. Unfortunately there has been no further action taken by Toyota to remedy this issue, although they are fully aware of it. Hoping you get more complains on this matter and force Toyota to take action. The vehicle would not accelerate above 20 mph and the RPMs would go up.  No lights or warnings were produced.   The only way to get the vehicle to act normally once again was to pull the car over into a safe location, shift the car into park, shut the car off, wait for a few seconds, restart the car and proceed.  In one session (driving the same day) the car repeated the inability to accelerate each time the vehicle was slowed down below 40 mph to come to a stop at a stop sign, take a turn, stop at a traffic light etc. This was dangerous as I was not able to proceed in traffic and caused other drivers to honk their horns at me, and left me stuck in intersections.  This was very disconcerting and left me feeling endangered by not being able to follow the flow of traffic. Toyota Service did a diagnostic test on it twice now.  The first time had torque converter codes.  This 2nd time they claim there were no codes on the diagnostic test. I contacted Toyota and an investigation was conducted using Bosch Crash Data Retrieval Tool 23.2.1.  The data shows a 5 second timeframe with .50 second intervals.  The vehicle speed at the 5 second mark was 6.8mph with 0% accelerator pedal. At 2 seconds, the speed was 5.0mph with 8.5% accelerator pedal.  And, at the crash point the vehicle speed was 12.4mph with 100% accelerator pedal.   Toyota product liability contacted me with their report stating that there was no brake pedal application prior to the impact and that pressure was applied to the accelerator pedal at wide open throttle seconds prior to the impact, and they found no obstructions or interference with the operation of the accelerator pedal. Further, they found the brakes to be operational However, the vehicle could not be test test driven as it would not turn on. I am highly concerned this just occurred as this could cause a great accident or injury.   I have 1,100 miles on the car. It was purchased brand new.   Another issue I have been experiencing is when I engage the gas pedal, it does not accelerate and takes a few seconds to accelerate causing my forward momentum to stall out, this is an issue when I have been trying to merge into traffic and my car does not drive as I have anticipated. First one is fine, back one exploded. All around the edges is a very distinct crack in a connected line. There was minimal traffic, and I was the only other vehicle on the interstate with a good 4 car length between me and the next car which was 2 lanes over. Toyota claims it was a piece of debris or rock that hit it so they say it is not under warranty. $14k repair price which is almost a quarter of what I am paying for the vehicle. I pulled into a parking lot, put it in park and all of the sudden my car starting stuttering/jerking/jolting the whole car, the rpm's were fluttering up and down between 2-3, the car engine turned on and off and car felt very weird. Check engine light and yellow explanation symbol came on with message \" Engine stopped steering power low\" and turned car off. Then I turned it back on and it was doing the same thing jolting me forward with the same message and also the message \"Not Ready to drive\" and shut the whole car back off. I could barely make it out of the drive thru to park it out of the way in a spot to wait for a tow truck. It wouldn't drive. I had to get it towed to dealership and of course when it got there it was acting normal. I got it brand new 3 weeks ago. The dealership had it for just over a day and said nothing was wrong and no codes were showing that anything happened. I am extremely concerned since something did happen! This seems dangerous especially since the codes did not record.
