Vibrates loose while driving.  It is a non-wear item.  Toyota had to recall 2009-2012 models of Avalon for the same problem.  My vehicle is a 2015 and is out of warranty and only has 50,000 miles.  I did have a service center check it and they confirmed the problem.  You can repair this non-wear item you must replace the complete steering column assembly.  With parts and labor the cost is @ $1500.  Whatever they did to redesign the mechanism since the 2012 model year to fix the problem was not effective.
