Apparently it is a known issue from this engine (both CT200h and Prius) suffer from this issue. Didn't realize there is a TSB but since way out of warranty and not a recall they offered to fix for $3500. Hard to see the leak as it is in the timing chain area so it rarely comes out of the engine itself. This should be something that Lexus/Toyota should advise their customers.
