Took to local Toyota Dealer and it was found that the Spiral Cable in the steering wheel was the cause and needed replaced so I had the dealer replace this Spiral Cable in the steering wheel which then fixed this issue. I found online that there were recalls and service bulletins on this issue, but it was for prior years and did not include my vehicle.
