Moments prior to the accident, she described what sounds like a brake booster failure.  There was no indication the AEB system activated and only the airbag at her feet deployed.  She hit the car in front of her at ~35 mph.  1. Brake system failure - yes, it is currently available for inspection 2. Car failed to stop and resulted in an accident.  Luckily no-one was seriously injured. 3&4 car has not be examined by anyone, yet.
