The inner brake pad material on the front wheel of the door side and off door side separated. The material did not come unbonded from the pad plate, the material itself separated. I have the pads and pictures of the failure.  How was your safety put at risk? The material could have become jammed in the brakes. This could have caused the single brake to lock up (possible loss of control), cause a brake to overheat to failure, or catch the tire on fire. The material could have totally come loose. This could have caused a brake line to be severed causing loss of brakes or, due to loss of pad material, a reduction in braking capability.  Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, the technician working on my vehicle stated he has seen this brake pad material failure on Kodiak pads multiple times.  Has the vehicle or component been inspected by the manufacturer? Not yet.
